Dac\u0103 <a href=\"https:\/\/www.roneuro.ro\/specialitati\/somnologie\/2-uncategorised\/88-insomnia.html\">somnul este perturbat<\/a>, mintea devine vulnerabil\u0103 la stres \u0219i\u00a0 anxietate [4-6].<\/p>\n<p>\u00cen\u021belegerea modului \u00een care interac\u021bioneaz\u0103 g\u00e2ndurile intruzive, ritualurile compulsive \u0219i insomnia poate ajuta la clarificarea motivului pentru care <a href=\"https:\/\/roneuro.ro\/blog\/primul-studiu-national-asupra-calitatii-somnului\/\">problemele de somn<\/a> sunt at\u00e2t de frecvente \u00een TOC \u0219i modul \u00een care \u00eembun\u0103t\u0103\u021birea somnului contribuie la gestionarea simptomelor [7].<\/p>\n<h2>G\u00e2nduri intruzive \u0219i ciclul de insomnie \u00een tulburarea obsesiv-compulsiv\u0103<\/h2>\n<p>G\u00e2ndurile intruzive reprezint\u0103 nucleul central al TOC [8-10]. Acestea [11] pot ap\u0103rea \u00een orice moment al zilei, dar noaptea se amplific\u0103 cel mai mult. Pacien\u021bii cu TOC, manifest\u0103 un val de g\u00e2nduri tulbur\u0103toare [12] tocmai atunci c\u00e2nd \u00eencearc\u0103 s\u0103 se relaxeze \u0219i s\u0103 adoarm\u0103.<\/p>\n<p>Aceast\u0103 hiperactivitate mintal\u0103 favorizeaz\u0103 insomnia [13]. \u00cencerc\u0103rile de a suprima sau neutraliza aceste g\u00e2nduri e\u0219ueaz\u0103, f\u0103c\u00e2ndu-le s\u0103 par\u0103 \u0219i mai\u00a0 amenin\u021b\u0103toare. \u00cen timp, creierul \u00eencepe s\u0103 asocieze noaptea \u0219i somnul cu frica [14] \u0219i lupta mintal\u0103 \u00eent\u0103rind insomnia, deoarece:<\/p>\n<ul>\n<li>Dificultatea de a adormi creeaz\u0103 frustrare \u0219i anxietate anticipativ\u0103 [15] legat\u0103 de somn.<\/li>\n<li>Pacientul anticipeaz\u0103 o experien\u021b\u0103 nelini\u0219tit\u0103, m\u0103rind starea de excitare \u0219i vigilen\u021b\u0103 [16].<\/li>\n<li>Excita\u021bia accentuat\u0103 alimenteaz\u0103 g\u00e2nduri intruzive, \u00eent\u00e2rziind \u0219i mai mult somnul [17].<\/li>\n<\/ul>\n<p>Prin urmare, insomnia este o consecin\u021b\u0103 a TOC, dar \u00een acela\u0219i timp este \u0219i o parte integrant\u0103 a modului \u00een care se men\u021bine tulburarea [18].<\/p>\n<h2>Resurse \u0219i suport pentru pacien\u021bi<\/h2>\n<p>Dac\u0103 observa\u021bi c\u0103 ave\u021bi un somn deficitar sau tulburat, o evaluare a somnului poate fi util\u0103 pentru a \u00een\u021belege cauzele \u0219i a primi recomand\u0103ri personalizate. \u00cen cazul pacien\u021bilor cu tulbur\u0103ri obsesiv-compulsive, aceast\u0103 evaluare poate ajuta la gestionarea simptomelor asociate. Acestea fragmenteaz\u0103 ciclul natural al somnului. \u00cen timp, obsesiile \u0219i compulsiile favorizeaz\u0103 [28]:<\/p>\n<ul>\n<li>Privarea cronic\u0103 de somn,<\/li>\n<li>Reducerea capacit\u0103\u021bii creierului de a regla emo\u021biile.<\/li>\n<\/ul>\n<h2>Cum agraveaz\u0103 problemele de somn simptomele tulbur\u0103rii obsesiv-compulsive?<\/h2>\n<p>O minte odihnit\u0103 poate recunoa\u0219te un g\u00e2nd intruziv ca pe un zgomot mental lipsit de sens, dar \u00een cazul insomniilor mintea este predispus\u0103 s\u0103 \u00eel interpreteze ca fiind periculos [29].<\/p>\n<p>Acest sentiment accentuat de amenin\u021bare alimenteaz\u0103 anxietatea \u00eent\u0103rind impulsul de a exercita compulsii. Anxietatea crescut\u0103 \u00eent\u0103re\u0219te nevoia de a efectua ritualuri [32].<\/p>\n<h3>Somnul deficitar reduce toleran\u021ba la incertitudine<\/h3>\n<p>Pacien\u021bii \u00ee\u0219i doresc o stare de certitudine \u0219i u\u0219urare a stresului, angaj\u00e2ndu-se \u00een comportamente compulsive. Prin repetare se \u00eent\u0103re\u0219te convingerea necesit\u0103\u021bii compulsiilor pentru a face fa\u021b\u0103 tulbur\u0103rii, agrav\u00e2nd tulburarea [33].<\/p>\n<h2>Cum s\u0103 crea\u021bi o rutin\u0103 s\u0103n\u0103toas\u0103 de somn atunci c\u00e2nd ave\u021bi tulburare obsesiv-compulsiv\u0103?<\/h2>\n<p>Crearea unei rutine s\u0103n\u0103toase de somn este posibil\u0103 \u0219i benefic\u0103 pentru paci. Pentru aceasta e nevoie de preg\u0103tirea unui ambient c\u00e2t mai favorabil somnului, lini\u0219te, lumin\u0103 slab\u0103, temperatur\u0103 u\u0219or r\u0103coroas\u0103, pl\u0103cut\u0103. Cele mai importante strategii [34] sunt urm\u0103toarele:<\/p>\n<ul>\n<li>Men\u021bine\u021bi intervale orare regulate de somn \u0219i trezire, favoriz\u00e2nd adormirea natural\u0103;<\/li>\n<li>Programarea \u201etimpului pentru griji\u201d mai devreme \u00een cursul serii;<\/li>\n<li>Limita\u021bi ritualurile nocturne;<\/li>\n<li>Accepta\u021bi existen\u021ba g\u00e2ndurilor cu minim\u0103 implicare \u00een ele;<\/li>\n<li>Face\u021bi c\u00e2teva exerci\u021bii de respira\u021bie.<\/li>\n<\/ul>\n<p><strong>Men\u021binerea unor intervale orare regulate de somn \u0219i trezire<\/strong> poate preveni perturbarea ulterioar\u0103 a ciclului de somn [35].<\/p>\n<p><strong>Programarea \u201etimpului pentru griji\u201d mai devreme \u00een cursul serii <\/strong>poate oferi un spa\u021biu desemnat pentru a recunoa\u0219te preocup\u0103rile, astfel \u00eenc\u00e2t acestea s\u0103 se simt\u0103 mai pu\u021bin intruzive la culcare. G\u00e2ndurile intruzive pot fi abordate prin acceptare [36].<\/p>\n<p><strong>Limitarea ritualurilor nocturne<\/strong> este dificil\u0103 dar reducerea treptat\u0103 face acest proces mai u\u0219or de gestionat. De exemplu, o persoan\u0103 care verific\u0103 \u00eencuietorile de mai multe ori ar putea reduce num\u0103rul de verific\u0103ri sau le poate am\u00e2na [23].<\/p>\n<p><strong>Acceptarea existen\u021bei g\u00e2ndurilor<\/strong> este o abordare aliniat\u0103 cu <a href=\"https:\/\/roneuro.ro\/blog\/suportul-psihologic-pentru-pacientii-neurologici\/\">strategiile cognitiv-comportamentale<\/a> [37], care reduce starea de alert\u0103 mintal\u0103. \u00cen timp, creierul \u00eenva\u021b\u0103 c\u0103 somnul este posibil f\u0103r\u0103 ritualuri ample, sl\u0103bind asocierea dintre ora de culcare \u0219i comportamentul compulsiv.<\/p>\n<p><strong>Exerci\u021biile de respira\u021bie<\/strong> reduc anxietatea prin activarea r\u0103spunsului de relaxare, cele mai cunoscute fiind tehnica respira\u021biei complete yoghine [38], respira\u021bia diafragmatic\u0103 \u0219i altele.<\/p>\n<blockquote><p>De\u0219i aceste strategii nu vindec\u0103 TOC, ele pot \u00eembun\u0103t\u0103\u021bi semnificativ calitatea somnului, favoriz\u00e2nd o gestionare mai bun\u0103 a simptomelor.<\/p><\/blockquote>\n<h2>Concluzii<\/h2>\n<p>Leg\u0103tura dintre somn \u0219i tulburarea obsesiv-compulsiv\u0103 este complex\u0103, intim\u0103 \u0219i bilateral\u0103. G\u00e2ndurile intruzive \u0219i compulsiile perturb\u0103 calitatea \u0219i durata somnului. Compulsiile, \u00een special ritualurile nocturne, perturb\u0103 tiparele de somn \u0219i \u00eempiedic\u0103 odihna restauratoare.<\/p>\n<p>Recunoa\u0219terea acestei rela\u021bii bidirec\u021bionale este esen\u021bial\u0103 pentru a \u00een\u021belege de ce problemele de somn sunt at\u00e2t de frecvente \u00een TOC \u0219i de ce abordarea somnului are un rol semnificativ \u00een recuperare.<\/p>\n<p>\u00cembun\u0103t\u0103\u021birea somnului nu elimin\u0103 obsesiile sau compulsiile \u00een sine, ci \u00eent\u0103re\u0219te resursele mintale necesare pentru a le face fa\u021b\u0103.
