Privarea sau fragmentarea somnului m\u0103re\u0219te nivelurile de cortizol nocturn, contribuind la stres cronic, inflama\u021bie \u0219i leziuni vasculare (factori de risc pentru AVC) [8].<\/p>\n<h3>Metabolismele glucozei \u0219i lipidelor \u0219i rezisten\u021ba la insulin\u0103<\/h3>\n<p>Somnul insuficient este corelat cu perturbarea <a href=\"https:\/\/www.roneuro.ro\/specialitati\/endocrinologie.html\">metabolismului<\/a> glucozei [9], dezvoltarea rezisten\u021bei la insulin\u0103 [10] \u0219i riscul apari\u021biei diabetului zaharat de tip 2, care contribuie la AVC.<\/p>\n<p>Tulbur\u0103rile de somn modific\u0103 metabolismul lipidic [11], m\u0103rind nivelul colesterolului \u0219i riscul de ateroscleroz\u0103.<\/p>\n<h3>Reglarea inflama\u021biei<\/h3>\n<p>Privarea cronic\u0103 de somn favorizeaz\u0103 cre\u0219terea markerilor inflamatori [12]. Leziunile cerebrale afecteaz\u0103 regiunile responsabile cu reglarea somnului, precum trunchiul cerebral \u0219i hipotalamusul [16].<\/p>\n<p>\u00cen aceast\u0103 faz\u0103, somnul poate p\u0103rea normal, dar\u00a0 nu este restaurator. Astfel, pacien\u021bii au perioade lungi de somn, dar se simt obosi\u021bi. Aceast\u0103 perturbare influen\u021beaz\u0103 negativ stabilitatea neurologic\u0103, complic\u00e2nd recuperarea [17].<\/p>\n<h3>Faza cronic\u0103<\/h3>\n<p>Pe m\u0103sur\u0103 ce pacien\u021bii intr\u0103 \u00een faza cronic\u0103 (c\u00e2teva luni post-AVC), tulbur\u0103rile de somn persist\u0103 sau chiar se agraveaz\u0103. Ele sunt\u00a0 influen\u021bate de leziuni neurologice \u0219i factori <a href=\"https:\/\/www.roneuro.ro\/specialitati\/psihologie.html\">psihologici<\/a> (<a href=\"https:\/\/roneuro.ro\/blog\/ce-este-depresia\/\">depresie<\/a> \u0219i <a href=\"https:\/\/roneuro.ro\/blog\/care-sunt-principalele-tulburari-de-anxietate\/\">anxietate<\/a>), care sunt frecvente post-AVC.<\/p>\n<blockquote><p>Insomnia cronic\u0103 are un efect negativ asupra dizabilit\u0103\u021bii \u0219i revenirii la locul de munc\u0103 la un an post-AVC [18]. Somnul deficitar persistent, \u00een faza cronic\u0103 post-AVC, este asociat cu riscul de recidiv\u0103 al AVC \u0219i complica\u021bii cardiovasculare [19].<\/p><\/blockquote>\n<h2>Care tulburari de somn cresc riscul de AVC?<\/h2>\n<p>Tulbur\u0103rile de somn reprezint\u0103 factori de risc independen\u021bi \u0219i modificabili pentru AVC precum apneea obstructiv\u0103 \u00een somn [20], insomnia, durata somnului [21] \u0219i tulbur\u0103rile de ritm circadian.<\/p>\n<h3>Apneea obstructiv\u0103 \u00een somn<\/h3>\n<p><a href=\"https:\/\/roneuro.ro\/blog\/sindromul-de-apnee-in-somn\/\">Apneea obstructiv\u0103 \u00een somn<\/a> (care apare \u00een faza acut\u0103 post-AVC) [22] este caracterizat\u0103 prin episoade repetate de obstruc\u021bie a <a href=\"https:\/\/www.roneuro.ro\/specialitati\/somnologie\/2-uncategorised\/89-tulburarile-respiratorii-din-timpul-somnului.html\">c\u0103ilor respiratorii<\/a> \u00een timpul somnului, hipoxie intermitent\u0103 \u0219i treziri frecvente favoriz\u00e2nd apari\u021bia:<\/p>\n<ul>\n<li><a href=\"https:\/\/roneuro.ro\/blog\/ce-este-hipertensiunea-arteriala\/\">Hipertensiunii arteriale<\/a> [23],<\/li>\n<li>Modificarea func\u021biei stratului intern al vaselor de s\u00e2nge [24],<\/li>\n<li>Cre\u0219terea activit\u0103\u021bii simpatice [25] (<a href=\"https:\/\/roneuro.ro\/blog\/sindromul-de-tahicardie-posturala-ortostatica\/\">tahicardie<\/a>, vasoconstric\u021bie, etc),<\/li>\n<li>Inflama\u021biei sistemice (r\u0103spuns imunitar generalizat, cronic, afect\u00e2nd tot corpul) [26].<\/li>\n<\/ul>\n<p>Toate acestea m\u0103resc riscul de AVC <a href=\"https:\/\/roneuro.ro\/blog\/cum-recunoastem-semnele-unui-avc\/\">ischemic<\/a> \u0219i hemoragic. \u00cen plus, <a href=\"https:\/\/roneuro.ro\/blog\/apneea-in-somn\/\">apneea obstructiv\u0103 \u00een somn<\/a> este deseori nediagnosticat\u0103 \u00eenainte de eveniment.<\/p>\n<h3>Insomnia<\/h3>\n<p><a href=\"https:\/\/www.roneuro.ro\/specialitati\/somnologie\/2-uncategorised\/88-insomnia.html\">Insomnia<\/a> manifestat\u0103 prin dificultate \u00een ini\u021bierea sau men\u021binerea <a href=\"https:\/\/roneuro.ro\/blog\/legatura-dintre-somn-si-anxietate\/\">somnului<\/a>, este asociat\u0103 cu\u00a0 riscul crescut de AVC [27], deoarece favorizeaz\u0103 [28]:<\/p>\n<ul>\n<li>Niveluri crescute ale hormonilor de stres,<\/li>\n<li>Cre\u0219terea tensiunii arteriale,<\/li>\n<li>Capacitate limitat\u0103 de recuperare vascular\u0103.<\/li>\n<\/ul>\n<blockquote><p>Comportamentele \u0219i obiceiurile nes\u0103n\u0103toase, precum lipsa activit\u0103\u021bii fizice \u0219i dieta incorect\u0103, agraveaz\u0103 \u0219i mai mult riscul de AVC.<\/p><\/blockquote>\n<h3>Hipo si hipersomnie<\/h3>\n<p>At\u00e2t somnul scurt (sub 6 ore pe noapte), c\u00e2t \u0219i somnul excesiv de lung (peste 9 ore pe noapte) sunt asociate riscului crescut de AVC [29], deoarece somnul scurt contribuie la disfunc\u021bia metabolic\u0103 \u0219i <a href=\"https:\/\/roneuro.ro\/blog\/principalele-teste-cardiace\/\">cardiovascular\u0103<\/a> [30], \u00een timp ce somnul lung (deseori fragmentat [31]) creeaz\u0103 inflama\u021bie sistemic\u0103 de grad sc\u0103zut, deregl\u0103ri metabolice (inclusiv rezisten\u021b\u0103 la insulin\u0103 \u0219i risc crescut de diabet de tip 2), tulbur\u0103ri afective (\u00een special depresie), reducerea activit\u0103\u021bii fizice \u0219i un risc crescut de boli cardiovasculare \u0219i mortalitate general\u0103.<\/p>\n<h3>Tulbur\u0103rile de ritm circadian<\/h3>\n<p><a href=\"https:\/\/www.roneuro.ro\/specialitati\/somnologie\/2-uncategorised\/90-tulburarile-de-ritm-circadian.html\">Tulbur\u0103rile de ritm circadian<\/a> (observate la angaja\u021bii care lucreaz\u0103 \u00een ture) m\u0103resc riscul de AVC prin modificarea ciclurilor hormonale, a tiparelor tensiunii arteriale \u0219i a proceselor metabolice [32].<\/p>\n<p><img creierului de a se reorganiza \u0219i de a forma noi conexiuni neuronale), iar acest proces este esen\u021bial pentru <a href=\"https:\/\/roneuro.ro\/blog\/recuperare-dupa-avc\/\">recuperarea post-AVC<\/a>.<\/p>\n<p>\u00cen timpul somnului cu unde lente \u0219i somnului REM (somn cu mi\u0219c\u0103ri rapide ale ochilor), creierul consolideaz\u0103 \u00eenv\u0103\u021barea, \u00eent\u0103re\u0219te conexiunile sinaptice, favoriz\u00e2nd recuperarea func\u021biilor motorii, cognitive \u0219i lingvistice \u0219i echilibrarea emo\u021bional\u0103 Din acest motiv, tratamentul tulbur\u0103rilor de somn trebuie s\u0103 cuprind\u0103 \u0219i aspectele cognitiv comportamentale [36]. \u00cembun\u0103t\u0103\u021birea calit\u0103\u021bii somnului are un impact pozitiv asupra st\u0103rii de spirit, motiva\u021biei \u0219i implic\u0103rii generale \u00een reabilitare.<\/p>\n<h2>Concluzii<\/h2>\n<p>Rela\u021bia dintre somn \u0219i AVC este complex\u0103 \u0219i bilateral\u0103. Somnul influen\u021beaz\u0103 riscul de AVC prin efectele sale asupra regl\u0103rii tensiunii arteriale, echilibrului hormonal, func\u021biei metabolice, inflama\u021biei \u0219i controlului sistemului nervos autonom.<\/p>\n<p>\u00cen urma unui AVC, tiparele de somn sunt\u00a0 modificate, cu provoc\u0103ri distincte at\u00e2t \u00een \u200b\u200bfaza acut\u0103, c\u00e2t \u0219i \u00een cea cronic\u0103.
