Vertijurile frecvente sau ame\u021belile constante pot fi un factor de \u00eengrijorare. Totu\u0219i, ame\u021belile rareori semnaleaz\u0103 o afec\u021biune care pune via\u021ba \u00een pericol.<\/p>\n<p>De cele mai multe ori ame\u021beala este trec\u0103toare \u0219i nu amenin\u021b\u0103 s\u0103n\u0103tatea \u00een niciun fel, nefiind o boal\u0103, ci un simptom. Mul\u021bi resim\u021bim ame\u021beala c\u00e2nd ne ridic\u0103m brusc, dac\u0103 nu am b\u0103ut suficient\u0103 ap\u0103, am stat mult \u00een c\u0103ldur\u0103 sau am uitat s\u0103 lu\u0103m masa.<\/p>\n<h2><strong>Cum se manifest\u0103 ame\u021beala?<\/strong><\/h2>\n<p>Ame\u021beala poate \u00eensemna lucruri diferite pentru fiecare dintre noi \u0219i poate fi ap\u0103rea din multiple motive \u0219i \u00een multiple situa\u021bii. Persoanele care se confrunt\u0103 cu ame\u021beli descriu senza\u021bia ca:<\/p>\n<ul>\n<li>O fals\u0103 senza\u021bie de mi\u0219care sau de \u00eenv\u00e2rtire (vertij)<\/li>\n<li>Senza\u021bie de le\u0219in<\/li>\n<li>Instabilitate sau o pierdere a echilibrului<\/li>\n<li>O senza\u021bie de plutire sau de \u201ccap u\u0219or\u201c<\/li>\n<li>Senza\u021bie de sl\u0103biciune, c\u0103 se \u00eenmoaie picioarele, c\u0103 se \u00eentunec\u0103 vederea sau de vedere \u00een cea\u021b\u0103.<\/li>\n<\/ul>\n<p>Aceste simptome pot fi declan\u0219ate sau \u00eenr\u0103ut\u0103\u021bite de mersul pe jos, de ridicarea \u00een picioare sau de mi\u0219carea capului. Ame\u021beala poate fi \u00eenso\u021bit\u0103 de grea\u021b\u0103 sau poate fi at\u00e2t de brusc\u0103 sau de sever\u0103 \u00eenc\u00e2t s\u0103 fie nevoie s\u0103 v\u0103 a\u0219eza\u021bi sau s\u0103 v\u0103 \u00eentinde\u021bi. Episodul poate dura c\u00e2teva secunde sau zile \u0219i se poate repeta.<\/p>\n<p style=\"text-align: left;\"><strong>Adresa\u021bi-v\u0103 medicului dumneavoastr\u0103 dac\u0103 ave\u021bi ame\u021beli sau vertijuri recurente, bru\u0219te, severe sau prelungite \u0219i inexplicabile!<\/strong><\/p>\n<p>Solicita\u021bi asisten\u021b\u0103 medical\u0103 de urgen\u021b\u0103 dac\u0103 prezenta\u021bi ame\u021beli sau vertij nou, sever, \u00eempreun\u0103 cu oricare dintre urm\u0103toarele:<\/p>\n<div style=\"border: 1px solid #6852ed; padding: 10px;\">\n<ul>\n<li>Durere de cap brusc\u0103, sever\u0103<\/li>\n<li>Dureri \u00een piept<\/li>\n<li>Dificult\u0103\u021bi de respira\u021bie<\/li>\n<li>Le\u0219in<\/li>\n<li>B\u0103t\u0103i rapide sau neregulate ale inimii<\/li>\n<li>Confuzie sau dificult\u0103\u021bi de vorbire<\/li>\n<li>Dificult\u0103\u021bi la mers<\/li>\n<li>O schimbare brusc\u0103 a auzului<\/li>\n<li>Amor\u021beal\u0103 sau sl\u0103biciune facial\u0103<\/li>\n<li>V\u0103rs\u0103turi dese sau persistente<\/li>\n<li>Senza\u021bie de vedere dubl\u0103<\/li>\n<li>Senza\u021bie de amor\u021beal\u0103 pe jum\u0103tate din corp sau fa\u021b\u0103<\/li>\n<li>Convulsii [3].<\/li>\n<\/ul>\n<\/div>\n<p>Ame\u021beala ca \u0219i simptom unic, mai ales dac\u0103 trece dup\u0103 c\u00e2teva momente, nu ar trebui s\u0103 fie un semn de \u00eengrijorare. Dac\u0103 solicita\u021bi un tratament, medicul \u00eel va baza pe cauza afec\u021biunii \u0219i pe simptome. Acesta poate include medicamente \u0219i exerci\u021bii de echilibru. Chiar dac\u0103 nu se g\u0103se\u0219te nicio cauz\u0103 sau dac\u0103 ame\u021belile persist\u0103, medicamentele prescrise \u0219i alte tratamente pot face ca simptomele s\u0103 fie mai u\u0219or de gestionat.<\/p>\n<p><strong>Medicamente:<\/strong><\/p>\n<ul>\n<li>Dac\u0103 ave\u021bi boala M\u00e9ni\u00e8re, medicul v\u0103 poate prescrie un diuretic. Acesta, \u00eempreun\u0103 cu o diet\u0103 s\u0103rac\u0103 \u00een sare, ajut\u0103 la reducerea frecven\u021bei episoadelor de ame\u021beal\u0103.<\/li>\n<li><strong>Medicamente care amelioreaz\u0103 ame\u021belile \u0219i grea\u021ba: <\/strong>de ex. antihistaminice \u0219i anticolinergice eliberate pe baz\u0103 de re\u021bet\u0103.<\/li>\n<li><strong>Medicamente anti-anxietate.<\/strong><\/li>\n<li><strong>Medicamente preventive pentru migren\u0103. <\/strong><\/li>\n<\/ul>\n<p><strong>Altele:<\/strong><\/p>\n<ul>\n<li><strong>Manevre de pozi\u021bionare a capului: <\/strong>O tehnic\u0103 numit\u0103 Manevra Epley poate ajuta \u00een cazul vertijului pozi\u021bional paroxistic benign.<\/li>\n<li><strong>Terapia de echilibru (reabilitarea vestibular\u0103):<\/strong> Exerci\u021biile specifice ajut\u0103 sistemul de echilibru s\u0103 fie mai pu\u021bin sensibil la mi\u0219care. Aceast\u0103 tehnic\u0103 este utilizat\u0103 pentru persoanele cu ame\u021beli cauzate de afec\u021biuni ale urechii interne (ex. nevrita vestibular\u0103).<\/li>\n<li><strong>Psihoterapie: <\/strong>Pentru persoanele ale c\u0103ror ame\u021beli sunt cauzate de tulbur\u0103ri de anxietate.<\/li>\n<li><strong>Proceduri chirurgicale<\/strong>.<\/li>\n<\/ul>\n<p>Dac\u0103 ave\u021bi o stare puternic\u0103 de ame\u021beal\u0103 cel mai rapid \u0219i u\u0219or lucru pe care-l pute\u021bi face este s\u0103 v\u0103 \u00eentinde\u021bi sau s\u0103 v\u0103 a\u0219eza\u021bi.<\/p>\n<p><strong>Evita\u021bi consumul de alcool, tutun, cafea \u0219i sarea \u00een exces deoarece acestea v\u0103 acentueaz\u0103 ame\u021beala!<\/strong><\/p>\n<p>Dac\u0103 a\u021bi observat c\u0103 ame\u021bi\u021bi la mi\u0219c\u0103ri bru\u0219te sau c\u00e2nd v\u0103 ridica\u021bi, \u00eencerca\u021bi s\u0103 v\u0103 mi\u0219ca\u021bi mai lent. Dac\u0103 ave\u021bi episoade dese de ame\u021beal\u0103 \u00eencerca\u021bi s\u0103 respecta\u021bi orele de somn \u0219i de mas\u0103 \u0219i ave\u021bi grij\u0103 s\u0103 be\u021bi suficiente lichide. Dac\u0103 ave\u021bi senza\u021bie de dezechilibru sau de c\u0103dere iminent\u0103 este bine s\u0103 urca\u021bi \u0219i s\u0103 cobor\u00e2\u021bi treptele sprijinindu-v\u0103 de balustrad\u0103. De asemenea \u00eencerca\u021bi s\u0103 \u00eendep\u0103rta\u021bi din cale obiectele de mobilier de care v-a\u021bi putea lovi. Este bine ca atunci c\u00e2nd face\u021bi du\u0219 sau baie s\u0103 folosi\u021bi covora\u0219e antiderapante \u00een cad\u0103 \u0219i \u00een vecin\u0103tatea acesteia.<\/p>\n<h2><strong>Este ame\u021beala acela\u0219i lucru cu vertijul? <\/strong><\/h2>\n<p><strong>Ame\u021beala (inclusiv vertijul) afecteaz\u0103 anual 15% p\u00e2n\u0103 la peste 20% dintre adul\u021bi<\/strong>. Prevalen\u021ba sa cre\u0219te odat\u0103 cu v\u00e2rsta \u0219i este de aproximativ dou\u0103-trei ori mai mare la femei dec\u00e2t la b\u0103rba\u021bi [1].<\/p>\n<p>Ame\u021beala \u0219i vertijul au un impact considerabil. \u00cen studiul epidemiologic din Germania, participan\u021bii cu vertij vestibular \u0219i ame\u021beli non-vestibulare au raportat consult medical (70% \u0219i 54%), concediu medical (41% \u0219i 15%), \u00eentreruperea activit\u0103\u021bilor zilnice (40% \u0219i 12%) \u0219i evitarea ie\u0219irii din cas\u0103 (19% \u0219i 10%). \u00cen plus, calitatea vie\u021bii, ajustat\u0103 \u00een func\u021bie de v\u00e2rst\u0103 \u0219i sex, a fost mai sc\u0103zut\u0103 la persoanele cu ame\u021beli \u0219i vertij [2].<\/p>\n<p>Sim\u021bul echilibrului depinde de aportul combinat al diferitelor p\u0103r\u021bi ale sistemului senzorial:<\/p>\n<ul>\n<li><strong>Ochii<\/strong>: determin\u0103 unde se afl\u0103 corpul \u00een spa\u021biu \u0219i cum se mi\u0219c\u0103 acesta<\/li>\n<li><strong>Nervii senzoriali<\/strong>: trimit mesaje c\u0103tre creier despre mi\u0219c\u0103rile \u0219i pozi\u021biile corpului<\/li>\n<li><strong>Urechea intern\u0103<\/strong>: g\u0103zduie\u0219te senzorii pentru detectarea gravita\u021biei \u0219i a mi\u0219c\u0103rilor \u00eenainte \u0219i \u00eenapoi.<\/li>\n<\/ul>\n<h2><strong>Care sunt factorii de risc care influen\u021beaz\u0103 apari\u021bia ame\u021belii?<\/strong><\/h2>\n<p>Ame\u021beala are multe cauze posibile, inclusiv tulbur\u0103ri ale urechii interne, r\u0103ul de mi\u0219care \u0219i efectele medicamentelor. Uneori este cauzat\u0103 de o afec\u021biune de baz\u0103, cum ar fi circula\u021bia deficitar\u0103, o infec\u021bie sau o accidentare.<\/p>\n<h3><strong>V\u00e2rsta<\/strong><\/h3>\n<p>Adul\u021bii mai \u00een v\u00e2rst\u0103 sunt mai predispu\u0219i s\u0103 aib\u0103 afec\u021biuni medicale care provoac\u0103 ame\u021beli, \u00een special o senza\u021bie de dezechilibru sau s\u0103 ia medicamente care pot provoca ame\u021beli.<\/p>\n<h3><strong>Un episod anterior de ame\u021beal\u0103<\/strong><\/h3>\n<p>Cauzele pot fi multiple. O sc\u0103dere dramatic\u0103 a tensiunii arteriale sistolice poate avea ca rezultat o scurt\u0103 ame\u021beal\u0103 sau o senza\u021bie de le\u0219in. Aceasta poate ap\u0103rea dup\u0103 ce v-a\u021bi a\u0219ezat sau v-a\u021bi ridicat prea repede \u00een picioare, fiind cunoscut\u0103 ca <strong>hipotensiune ortostatic\u0103<\/strong>.<\/li>\n<li><strong>Circula\u021bia sanguin\u0103 deficitar\u0103.<\/strong> Afec\u021biuni precum cardiomiopatia, atacul de cord, aritmia cardiac\u0103 \u0219i atacul ischemic tranzitoriu ar putea provoca ame\u021beli. Probleme de circula\u021bie asociate ame\u021belii<\/p>\n<h3><strong>Alte cauze<\/strong><\/h3>\n<ul>\n<li><strong>Afec\u021biuni neurologice.<\/strong> Boala Parkinson \u0219i scleroza multipl\u0103 pot duce la pierderea progresiv\u0103 a echilibrului.<\/li>\n<li><strong>Anumite medicamente. <\/strong>De exemplu medicamentele anticonvulsive, antidepresive, sedative, tranchilizante, antihipertensive.<\/li>\n<li><strong>Tulbur\u0103ri de anxietate. <\/strong>Atacurile de panic\u0103 \u0219i teama de a p\u0103r\u0103si casa sau de a se afla \u00een spa\u021bii mari \u0219i deschise (agorafobia) pot provoca o senza\u021bie de ame\u021beal\u0103.<\/li>\n<li><strong>Niveluri sc\u0103zute de fier (anemie). <\/strong>Alte simptome care pot ap\u0103rea \u00eempreun\u0103 cu ame\u021belile includ oboseala, sl\u0103biciunea \u0219i pielea palid\u0103.<\/li>\n<li><strong>Nivel sc\u0103zut de zah\u0103r \u00een s\u00e2nge (hipoglicemie).<\/strong> Apare, \u00een general, la persoanele cu diabet zaharat care utilizeaz\u0103 insulin\u0103.
